Transition programs to guide incarcerated youth and adults back to the community are analyzed. Vital transition program components are described along with common characteristics of an effective transition coordinator. The transition program overview talks about: providing instruction through family literacy, Action for Personal Choice (APC), group and individual counseling, life skills, and substance abuse treatment; promoting job readiness; and creating a supportive network with peer support groups and drop-in centers. The effective transition coordinator meets the client's needs by utilizing case management and parental and family involvement while also meeting the needs of the community. A glossary of acronyms and a list of learning standards are also included.
